Pro Hac Vice Forms

The forms for requesting admission pro hac vice have been modified to facilitate electronic filing. Local counsel may now file a motion to admit the pro hac vice attorney and attach as exhibits the completed application of the pro hac vice attorney and the proposed order. With the inclusion of Pay.gov functionality to CM/ECF, local counsel can pay the $15.00 admission fee by credit card over the internet at the time of filing the motion.

Pro hac vice attorneys are encouraged to submit their registration for electronic filing when the motion is filed so that they can be added to the case as electronic filers when the court grants the motion for pro hac vice admission. The form may be the third attachment to the motion.
